Family history

 

  1. Where possible, take a three generational family history including 1° relatives (children, siblings, parents) and 2° relatives (aunts, uncles and grandparents) on both sides of the family
  2. Note:
  3. Ethnic background (ancestry and culture)
  4. Adoption
  5. Age at diagnosis
  6. Age and cause of death
  7. Birth defects, stillbirths and miscarriages
  8. There are no insurance or privacy implications from taking a family history.
